What these numbers mean
Days in A/R (lower is better)
Total A/R at month end divided by average daily gross charges over the last three months. Every extra day is cash the practice has earned but cannot use. It is usually the first number a lender or a new managing partner asks for.
Initial denial rate (lower is better)
Claims denied on first submission divided by claims submitted in the month, as a percent. Each denial is rework: someone has to find the reason, fix the claim and send it again. A rising rate is usually a front desk or coding problem that is cheaper to fix early.
No-show rate (lower is better)
No-show appointments divided by all booked appointments in the month, as a percent. An empty slot is a visit that cannot be billed and a patient who did not get care. Reminder calls and overbooking rules are tuned against this number.
Visits per provider per day (higher is better)
Completed visits divided by provider clinic days worked in the month. It is the clearest read on capacity. A low number with a long waiting list usually points at scheduling templates, not at the providers.
Net collection rate (higher is better)
Payments divided by (charges minus contractual adjustments), as a percent. This is the money left on the table after the contracts are honored. Well run practices sit in the high nineties; every point below is revenue already earned and never received.
Median, middle half, top and bottom quarter
The median is the middle site that month. The middle half runs from the site a quarter of the way up to the site three quarters of the way up. Top quarter means better than three quarters of the organization's sites, whichever way the number runs; bottom quarter means worse than three quarters of them.
