Where to look first

Bottom quartile of the organization's sites, or worse than its median and still getting worse.

  • Initial denial rate: 10.6%

    Bottom quartile, 3.0 points from the organization median, and worse than three months ago. Each denial is rework: someone has to find the reason, fix the claim and send it again. A rising rate is usually a front desk or coding problem that is cheaper to fix early.

    What to try first

    • Pull last month's denials and group them by reason and by payer; the top three reasons usually explain most of the total.
    • Confirm insurance eligibility and coverage for every patient before the visit, not after the claim comes back.
    • Have a coder review a sample of claims for the five most billed visit codes to catch errors that repeat.
  • No-show rate: 8.9%

    Bottom quartile, 2.3 points from the organization median. An empty slot is a visit that cannot be billed and a patient who did not get care. Reminder calls and overbooking rules are tuned against this number.

    What to try first

    • Look at which days, times and appointment types are missed most often; the pattern is usually narrow.
    • Send a reminder two days before each visit and a second one the day before, by text where patients have agreed to it.
    • Keep a short list of patients who can come in at short notice, so a missed slot can still be filled.
  • Visits per provider per day: 19.5 visits

    Bottom quartile, 2.2 visits from the organization median. It is the clearest read on capacity. A low number with a long waiting list usually points at scheduling templates, not at the providers.

    What to try first

    • Compare each provider's template with the visits actually booked to find slots that are blocked or rarely used.
    • Time a handful of visits from arrival to departure to see where rooms and staff sit idle.
    • Count the same day openings that go unfilled and check whether the front desk can book into them.
