The monthly site file
What your data team sends each month: one CSV file, one row per site, the first line exactly these column names in any order. No patient data is ever part of it.

The columns
- site_code
- The site's code in Practice Benchmark (for example MAIN), or the id the site has in your own system (for example DEPT-102), which is matched to the site. Send each site; regions and entities are refused, because their totals are worked out on screen.
- period
- The month, written year then month: 2026-09. A file holds one month, the month that just closed.
- days_in_ar
- Days in A/R. Total A/R at month end divided by average daily gross charges over the last three months. From 0 to 365.
- denial_rate
- Initial denial rate. Claims denied on first submission divided by claims submitted in the month, as a percent. From 0 to 100.
- no_show_rate
- No-show rate. No-show appointments divided by all booked appointments in the month, as a percent. From 0 to 100.
- visits_per_provider_day
- Visits per provider per day. Completed visits divided by provider clinic days worked in the month. From 0 to 60.
- net_collection_rate
- Net collection rate. Payments divided by (charges minus contractual adjustments), as a percent. From 0 to 100.
What the check does
- A blank value means the site did not report that number.
- A file with a missing or extra column is refused whole, so nothing loads by accident.
- A row is rejected, with a reason you can send back to the site, for an unknown or retired site, a region or entity, a site you do not load, the wrong month, a value that is not a number or is impossible, or two different rows for one site. An exact repeat is ignored.
- A number far outside the site's own last twelve months is held until a person confirms it.
- Loading a month again replaces a site's earlier row for that month. Every load is kept under the loader's name and cannot be edited or deleted.
